The information on this website applies to both the newer Underwood-Smith Teaching Scholars Program applications process and service obligation and graduates who received the former Underwood-Smith Teacher Scholarship bound by the original terms of the award at the time the time funds were distributed and a promissory note signed.
For Students Applying for the 2020-2021 Academic Year or Later
Applicants for the Underwood-Smith Teaching Scholars Program who attended private institutions or public institutions outside of West Virginia are required to submit a grade report form along with high school transcripts. This form is not required of students who attended a West Virginia public high school.

For Students Receiving Awards in 2019 or Earlier
If you received an award under the former Underwood-Smith Teacher Scholarship Program, please use the forms below to submit verification of your continued college enrollment or qualifying employment.
- Enrollment Verification Form for recipients who are still in school.
- Employment Verification Form for recipients who are fulfilling service obligations.
For Students Receiving Awards After 2019
